About Us

Support Solutions Healthcare Limited is a privately owned domiciliary care company with offices in Long Bennington Business Park. It was born out of personal experience and commitment to providing high quality care like we would wish our family members to receive.

Our personalised home care service gives you the choice to remain independent in your own home, without compromising on the quality of your care. Our geographical area covers the whole of Lincolnshire but with more clients in Grantham, Sleaford, Boston, Lincoln, Stamford and the surrounding villages.

Aims and Objectives

Aims:

Our aims at Support Solutions Healthcare Ltd are as follows:
• Enable and support clients to retain their independence of thought and activity.
• Meet the client's physical, emotional needs and overall well-being in a dignified non-judgmental way.
• Recognize the client’s diversity ensuring religious, cultural, racial and gender identities are respected
• Ensure that the client is always treated with dignity and respect and that they are safeguarded from the risk of abuse as outlined in the (Care Act 2014).
• Involve service users fully in planning their own care, devising, and implementing their care plans and managing the records of care.
• Ensure that all information received while carrying out our duties is treated with the utmost confidentiality and is not passed to any unauthorised persons. Records will be accurate and fit for purpose in line with (Data Protection Act 2018).
• Be respectful of the client’s personal privacy and space.
• Ensure that the quality of the service provided is of the highest standards possible and to use appropriate methods to achieve this.

Objectives:

We will fulfil our aims by adhering to the following objectives:
• Employing competent, fit for purpose staff and ensuring Care Staff are Care Certificated and QCF trained. Staff will receive specialised ongoing training to demonstrate achievement of the required standards and competencies.
• Ensure that all health and safety procedures are always followed.
• Obtaining informed consent from the Client for all aspects of care activities.
• Protecting physical privacy and dignity during care processes through careful planning and positive communication and attitude.
• Ensuring that complaints are listened to and responded to within given timescales.
• Supporting Clients to develop their social, vocational and recreational skills.
• To match the nominated Care Worker as closely as possible with the client and respecting the need to change the Care Worker in the event of subsequent non -compatibility.
• Involve service users and carers in the provisions, management, and development of services, which will be monitored regularly as part of the quality assurance framework.
• We understand that care needs are always subject to change, and the service we provide must be flexible to accommodate the changes without compromising the quality of care provided.

Person Centred Support

We aim to provide a dynamic, creative and high quality service that meets and exceeds the service user’s requirements, through person-centred support. Our objective is to deliver the highest standard of care and support, taking full consideration of the outcomes expected by the service users and the contracting authority. The objective is underpinned and achieved under a framework of principles of person centred care approaches that are based around our ability to offer:

Person Centred Individual Support Plans

The service user is fully involved in drawing up a Care and Support plan that meet and answers their own specific and particular requirements and outcomes. We will continually seek to improve and develop our service and will ensure we clearly identify roles and responsibilities within our organisation to manage the performance of our delivery including:

• Evidence based tool to set, track and measure impact as well as required change for achieving recovery and progression outcomes.
• Approach that ensures all tools are accessible with the capacity to support service users to be central and engaged with setting, measuring and celebrating quality of life goals and achievements as agreed in Individual Support Plans.
• Our Registered Manager and Management Team will conduct visits to service users to hear their views first hand.
• Our approach recognises and incorporate the role and importance of carers and family members
• Continuous reviews of Individual Support Planning and Risk Assessments with Individual Care and Support Plans.
• Fully compliant policy for managing challenging behaviour, taking into consideration all relevant legislation and guidance.
• Positive Behaviour Support Framework to assess the broad social, physical and individual contexts for behaviour occurring.• Palliative care
